BAL is performed via bronchoscopy under general anesthesia. Sterile saline (typically 2-4 aliquots of 20-25 mL each) is instilled into a lung segment and recovered by gentle aspiration. Return volumes are typically 40-60% of instilled volume. The procedure is well-tolerated and can be performed serially.
Typical return is 10-50 mL depending on instilled volume and technique. Standard protocol uses 50-100 mL total instillation. Both cellular (cell pellet) and acellular (supernatant) fractions are available. Custom protocols (targeted segments, specific volumes) can be arranged.
BAL fluid is evaluated for return volume, clarity, and differential cell count (when cellular fraction is requested). Total protein concentration and cell viability data are available. Samples with significant blood contamination (traumatic tap) are excluded from standard inventory.
